Can dissecting cellulitis affect scalp that still looks normal? In our paired-biopsy study of 12 men, PIILIF was present in 12/12 clinically normal-appearing scalp biopsies and 3/3 inactive nodules. Active lesions showed additional pus-forming inflammation and destructive change. Taken together, the findings suggest PIILIF may be a precursor stage of dissecting cellulitis. Confirming this relationship could open new therapeutic pathways aimed at earlier intervention, with the potential to limit permanent damage and reduce relapses more effectively.
