When John first arrived at my clinic, he described painful bumps from clippers and patches of hair loss that had been troubling him for years. But as I began assessing his scalp, it became clear that his symptoms were tied to something far more complex than simple irritation or inflammation. His scalp told a layered story, shaped by three conditions working together over a long period of time. At the back of his scalp, Acne Keloidalis Nuchae (AKN) had created a firm, raised ridge that had gradually thickened and expanded. This condition often begins with small bumps triggered by friction or close haircuts, but without early intervention it can evolve into dense, keloid-like tissue that becomes painful and emotionally distressing. Across the top of his scalp, I saw signs of Dissecting Cellulitis of the Scalp. This condition forms deep inflammatory tunnels, soft folds, draining bumps, and chronic swelling. It reshapes the surface of the scalp from the inside out, creating tenderness, drainage, and long-term hair loss. The tunnels and folds visible on John’s scalp were the result of years of inflammation beneath the surface. Layered throughout these changes were the deep grooves and folds characteristic of Cutis Verticis Gyrata. This condition alters the natural architecture of the scalp, creating wave-like ridges and thickened skin that add even more complexity to what the patient is experiencing. Seeing all of these conditions together felt like reading a map, each region marked by its own history of struggle. One condition built upward. Another burrowed inward. Another reshaped the foundation. Together, they revealed the physical and emotional weight that John had carried long before stepping into my office. Mapping his scalp was not simply a medical evaluation. It was a way to understand his journey, his unanswered questions, and the frustration he had lived with for so long. Every fold, tunnel, ridge, and bump represented years of discomfort and uncertainty. By identifying the origins of these changes, we were finally able to give him clarity and begin a restoration plan grounded in both science and compassion. If you or someone you know is experiencing painful bumps, scalp folds, tenderness, drainage, or unexplained hair loss, this episode may help you recognize the signs and understand that you are not alone. There is always a path forward, and healing begins with understanding.
